The goal of the Case Management Department at STSSH is designed to achieve optimal health, access to care and appropriate utilization of resources during and after the hospital stay.

Functions of Service:

• Care Coordination- Screening, Assessment, Plan of Care, Sequencing & Communication
• Compliance
• Transition Management
• Utilization Management

Patient Population:

Case Management services are available to all patients at STSSH. Case Management’s primary focus of coordination of care includes inpatient and observation hospital stays but is not limited to Outpatient and/or ER visits.
