Orthopaedic Nurse Navigator

The Orthopaedic Nurse Navigator assists Total Joint Arthroplasty patients and their families by acting as a liaison for patients and their families through the entire continuum of care by providing:

Functions of Service:

  • Pre-Operative Total Joint Replacement education for patients and their families to prepare them and improve outcomes for surgery.
  • Patient Care Coordination with Physicians, Managers, Nursing Staff, Case Management and Physical Therapists.
  • Improves communication of care among providers and the patient
  • Will round on Total Joint Replacement patients while in the hospital

The role includes acting as a resource for nursing staff for routine and complex problem solving, education, and continuous performance improvement efforts.

Patient Population:

Orthopaedic Nurse Navigator services are available to Total Joint Arthroplasty patients at STSSH. The Nurse Navigator’s primary focus is on Inpatient and Observation hospital stays but not limited to Outpatient and/or ER visits.

Hospital Staffing:

A full-time Nurse Navigator is available Monday-Friday 8:00 AM- 4:00 PM. The Nurse Navigator is also available weekends upon request or as needed.
